Don't sweat it. I used to have a rule that you get a flub (all 1's) when using one, you overdrove the persons imune system to where he worsened....

I go for the if you roll 10 or more below the target number you worsen the patient. On the other hand, I pretty harsh about what happens if you dont get any medical aid at all, so its never 'better' to go without aid.
