Die Code Simplification chart?

Anyone have a copy of the WEG Die Code Simplification chart?

The chart is basicly a criss cross of +4, +3, +4, +3 for each dice. You can roll five dice and "chart" the rest, or just chart the whole thing (except the wild die).
